Flu, RSV, and COVID shots this fall: what they actually cost with and without insurance

Fall vaccine season means three separate decisions for a lot of households: flu, RSV, and COVID-19. The cost picture for each is different enough that "how much will this cost me" doesn't have one answer — it depends heavily on which vaccines you're getting and whether you're insured.

Flu shot cost without insurance 2026

A standard quadrivalent flu shot runs $20 to $75 at a retail pharmacy without insurance, with a national median cash price around $45. The high-dose or adjuvanted version recommended for adults 65 and older costs more — $60 to $130. Major chains price toward the higher end of that range: Walgreens lists a standard shot at $58.99 and a senior/high-dose shot at $108.99, while CVS lists $68.99 and $128.99 respectively.

RSV vaccine cost without insurance

RSV is the most expensive of the three by a wide margin — $300 to $550 without insurance, with an average retail price around $350-$375. Both approved RSV vaccines carry manufacturer list prices just over $320 per dose (Arexvy at $321.05, Abrysvo around $320), before any pharmacy markup or administration fee.

Are flu and COVID vaccines free with insurance 2026?

For most insured people, yes. Under ACA Section 2713, non-grandfathered health plans are required to cover ACIP-recommended vaccines with no cost-sharing — and the insurance industry trade group AHIP has said flu, updated COVID-19, and RSV vaccines will be covered this way through the end of 2026. The exceptions worth knowing about: grandfathered plans, some short-term insurance plans, and out-of-network administration may not follow this rule, so it's worth a quick check with your specific plan if you're unsure.

COVID vaccine price 2026 uninsured

Without insurance, an updated COVID-19 vaccine can run up to roughly $190 at full retail price. That said, federal and state access programs have helped some uninsured adults get updated COVID-19 vaccines at no direct charge through participating pharmacies — worth asking about locally before assuming you'll pay full price.
